Portal quick reference

The screens in the order you will actually meet them. Every row says what the screen collects and what later step reads it, because that is the part the portal never shows you. The watch links are the official NHS England walkthrough videos; transcripts of all of them sit in this repository under docs-about/.

Evidence form Title, code, description, the summary box (why included, what it shows, leading or contributing or watching), attachments (five per item at most), the confirmation choice, and the 38 capability ticks. Copy the record's evidence link. Confirmation happens one of three ways: portal peer review, another organisation such as a university or professional body, or an editorial process such as a published paper.
